I have a client who is running a stand-alone copy of AREV 3.12 on an NT 4.0 workstation. He says that once it is running everything else comes to a crawl (utilization shoots up to 100%). Is there a configuration that can be implemented to avoid this?

I am not sure if this is any help, but we find that if AREV is left at a prompt that in effect is doing an INPUT X, -1 then windows cannot detect that it is in idle state. We have some menus and other programs that do not do this and therefore ease the problem. However, we have never used AREV indexes or menus or paint and therefore have more control than some might over this.
Have they set the "Yield time to Windows" flag in the hardware environment section?
