It's hard to believe I have never had to do this, but, then maybe its a tribute to OI.

If I create a new form, "no table" with a couple of text controls

and buttons how do I launch that form from a stored procedure and

If you want a non-modal display type box, use Create_Dialog.

If you are controlling properties from a stored procedure you

need to declare and use Get_Property and Set_Property
