Is it normal for the 3.03 upgrade diskette to fail a Scandisk check? Or is my diskette really corrupted?

As far as I know there is nothing unusual about the 3.03 upgrade in terms of scandisk (eg. hidden files or boot sector records). Looks like you have a bad disk - perhaps you can contact Revelation Software and they can issue you with a replacement.
Try installing it anyway on a test copy of your system - you never know - it may just work if the bad sectors are in an unused portion of the disk. Try several machines - I've seen cases where the diskette drive tolerances are out between machines just enough to get scandisk to complain.

The disk should be free of errors (no copy protection scheme).
There is a file verification option on the upgrade program as I recall.
The 3.03 maintenance release can be downloaded (3.0x to 3.03) from the Demos & Downloads section of the Web Site if your disk is indeed bad.
