I worked at a site where this was a problem to such an extent that we had to put in place a login procedure that checked the PIF. Only an authorized PIF was allowed. If sensors detected a change to the PIF, the user was denied entry and/or we replaced the PIF and forced a restart (I can't remember which). I will also add that this is a hard one to track who the culprit is, because the culprit likely isn't exhibiting any symptoms. It's the NEXT person needing the lock that has problems.

Sounds similar to the problem when someone minimizes AREV when an index is being updated and a log jam begins to happen as each other machine attempts to update the same index. Check your shortcuts to make sure that "Always suspend" is unchecked.
