Found the source of my problem:
The environment record for SYSPROG (and not the APP) didn't contain the Update Index Before Query flag (Field 6 in ENV_SYSPROG) was blank.
When the original deployment was made, the ENV_SPT record was copied from SYSPROG and not from the APP (which has several changes to the ENV record). This is in OI Version 3.7. I went through Don Bakke's suggestions on how to create a deployment entity and this fixes it.
