I have a client running Arev 3.12 on Win98 clients NT4.x server.
Is there any changes need to be made to AREV's or other setting.
They also run NPP 1.4. Could someone tell me also if any setting in NT land ie file caching need to be configured any particular way.

The first and most obvious issue I'd address is using the NT Service rather than just the NPP. This would generally render any OS settings irrelevant. Otherwise, just to be on the safe side, you will want to disable write behind caching on each workstation.
Performance can be enhanced by removing all unnecessary protocol to adapter bindings that don't get used.

Be careful with WIN98 and EMS. I believe the default for WIN95 is EMS is available but for WIN98 you must configure it in the CONFIG.SYS. We had such a problem at a client site and fixing the CONFIG.SYS allowed AREV to function.
