It sounds like there could be something wrong with the tables, but it is hard to tell.

I would suggest creating empty tables (presized) in a new volume and copying the data from the old tables to the new. Then get rid of (archive) the old.

Try finding one of the dictionary cleaner programs. You could have a few stray M1.1 or other older items in the dictionary.

You could also try clearing out all the information in the REVMEDIA records except for the DOS name and the MFS list (fields 1 and 2).
