REVDEBUG.DLL Errors (OpenInsight 16-Bit Specific)

One of our client's is experience random REVDEBUG.DLL errors. Judging from the previous posts, this has to do with the loss of the network connection and is more than likely, hardware related. But, despite our efforts, problems persists. Here is the run down.
OI V3.7.2
Windows 2000 Service 1.5 (tried 2.1 too)
Windows XP workstations on Dells with built-in network cards on MB.
Changed:
Replaced workstation card with 3com Ethernet xl 10/100 pci card ( 3c905c-tx).
Replaced CAT 5 cable to local switch A(3com -3c16471 - baseline 10/100 switch)
Replaced Switch A
Replaced cat5 cable from switch A to switch B (3com -3c16471 - baseline 10/100 switch).
Replaced switch B.
2000 Server plugs into switch B.
Doing a copy_row function to copy about 18000 rows will consistently produce the error on the workstation but not the server. Updating indexes, saving records, will occasionally give the error.
A third party vendor has tested the lines under maximum speed and bandwidth and did not receive any errors or diagnose any problems.
The error appears more often to heavier users than lighter ones, but light users do get the error occasionally.
Another interesting note: Once the error occurs, the user must shutdown the machine and do a cold restart, otherwise the network is not available.
Any other ideas?

get a better idea of the error.
Any chance that there is 1 machine with a bad card?
Are you running more than one network protocol?
As a complete shot in the dark, are there sizelock problems, especially on the table where you are doing the copy?
