Seemingly out of the blue AREV has started assigning key numbers that had been previously assigned and then the record was deleted. It has always just kept going up before. e.g. before 42000 - 42001 now 42000 - 29 - 79

Is this harmless, or cause for alarm?

That would depend upon your application and if there are any assumptions, for example, that the most recent key has the highest value. Most likely the next value was either lost (perhaps due to a read error on %SK%) or someone pressed=on the key field, prompting them to enter a new starting value.
