Attachtable Error (AREV Specific)

Its pretty clear there's some sort of problem connecting to tables. LH on each occasion looks OK but stop & restart no help. Only way to fix is to restore AREV directory, then reboot server and by default the LH service as well. The problem has occurred up to 6 times now over the last 4 weeks. Application running fine in between.
IT staff report that nothing unusual is running at the times of the crash - they are at different times etc. Usual suspects virus scanning etc have been excluded.
Crashed again last night and this time we have the following LH errors reported - there are 3 - same error different groups
Description:
The "LHWriteRecord" call failed, returning LH error number 104, file number 2, group number 90480. General write error.
Can anyone give me any ideas where else to look or what should be done with these errors? Client nearly going nuts!

Is quickattach being used? That often causes problems with the LH service or NLM. I forget where the attach images are stored, probably SYSENV/SYSTEM. Deleting the attach images is usually the quickest way to get rid of quickattach.
