Index Files Not Dynamically Updating (OpenInsight 32-Bit)

Hello again,
Well, it seems my system must be a repository for every glitch and problem that could possibly attack an application.
The latest and greates problem to arise is with indexing.
This is the third month in a row that when our accounting department ran their month-end reports, many of the indexed files were not indexed properly.
For example, our order history file which is a very simple file with only a few fields, was returning last months totals when the end of month report was run. I manually rebuilt the index on the customer number field and the report ran accurately. This particular file is only updated programmatically.
We have several forms that contain indexed, multi-value fields and they do not update automatically at all.
I no longer trust the integrity of our index files and desparately need any suggestions to try and remedy this situation.
I have tried to destroy and rebuild index files but this did not seem to help.
TIA for any help!

Charlie, you didn't say if indexing on these accounting tables used to work, and now don't, or have never worked.
If new indices, then are any a Symbolic? If a Symbolic that depends upon a value of a different table, then indexing doesn't work properly on such a symbolic.

There used to be a bug where transactions never got moved beyond the first level of distribution. Are they just sitting in the ! file?
