Has there been any changes to the OECGI in Oi 4.12(today's version) that I cannot get a OECGI request unless I'm running it in TCP/IP mode and connecting to a running application?

No, it doesn't. I use sambar on a workstation with everything under the hood.
I do get Fatal Runtime Error-] Sys1000:Error loading proram "inet_my_function"
CallStack-]RTP27, RUN_OECGI_REQUEST
Then I get this in my browser:
OECGI No reply received.
It almost seems like the oecgi and oengine have no rights in the application
