Does the error occur on one specific form or on numerous forms? If on numerous forms, is there something common to the forms (naming standard, et. al.) that could be causing an error?

This is caused by a MENU choice of the form not having any children. We found this error, and tracked it down. Mike R. knows of this, since I have told him.

We get around this by adding a child to the top level menu item and then hiding it.
